Please print and complete ALL FIVE pages and bring them with on the day of your scheduled appointment.
Please read the following information prior to your visit or consultation:
As a patient, it is your primary responsibility to ensure that we receive all of your pertinent medical information, test results, CT-Scans/X-ray reports, and other lab results.
All patients need to be sure that they bring a current list of ALL MEDICATIONS
1. It is very important that you clearly know the reason of your visit. That you have already made arrangements with your physician's office to send all pertinent reports, test results, and images to our office prior to your visit, or have them in your possession at the time of your visit.
If you have not, please contact your physician as soon as possible to make arrangements, so they can send your records to our office. Please call our office to confirm that we have received your records. If we have not, please call your physician and ensure that your records has been sent to us, or make arrangements to pick them up yourself prior to your visit. It is the patient's responsibility to see that the records and X-rays and scans are available. Current state and federal laws require that ONLY you, the patient, may obtain the records.
2. If your insurance company requires a referral, please make sure that your primary care physician has contacted your insurance company to arrange the referral before your scheduled appointment or you will be re-scheduled. It is the patient's responsibility to obtain a referral from the primary care physician. Co-pays, if required by your insurance, will be collected at the time of the visit. We do accept VISA and Mastercard - as well as cash or check payable to UROCARE - LLC.
3. You should fill out the registration forms which include necessary information about your general health. If you do not know the information required on the forms, it would be appropriate to find out this information from your primary physician's office. In most cases, your primary physician's nurses or medical assistants can help you answer many of these questions about prior diseases and current medications. You do not need to speak to your primary doctor personally.
4. If you have been a previous patient in our practice but have not been seen for three years or more, you will need to fill out new registration forms.
5. If you are coming in for a vasectomy consult, please download and print the vasectomy consult forms, sign the first page, and bring it with you on the day of your initial consultation with the doctor.
